ELIGIBILITY: Participation is open only to life science professionals 21 years and older in the US (excluding Puerto Rico) and Canada (excluding Quebec) who complete and submit a webform. Eligible participants will be entered into a drawing for a chance to win one Applied Biosystems™ MiniAmp™ Plus Thermal Cycler. Limit of one entry per person. The person submitting the webform is entering on behalf of their employer, who will be the recipient of the prize. Participant warrants that they have obtained any necessary permission to submit an entry to this sweepstakes and authorization from their employer or responsible supervisor.
Healthcare Professionals may not participate in this promotion. The term Healthcare Professionals includes clinical or non-clinical physicians (including MDs and MDs/Ph.D.s),and physician assistants, nurses, technicians, research coordinators, administrators and purchasing personnel and entities (including hospitals, rehabilitation centers, nursing facilities, home health agencies, clinics and group purchasing organizations, managed care organizations, physician group practices, medical directors for health insurance organizations) and their employees and representatives who are involved in the actual provision of health care services.

DURATION: The Applied Biosystems™ MiniAmp™ Thermal CyclerSweepstakes (the “Sweepstakes”)begins September 15, 2018. Completed webforms must be received by Sponsor no later than November 15, 2018 at 11:59 p.m. US Pacific Time to be included in the Sweepstakes.
WINNER: One winner (“Winner”) will be randomly selected on November 16, 2018 from all eligible entries received.The Winner will be notified by email, telephone, or mail within approximately 14 days after the drawing date. If the winner is unreachable or does not make direct contact with the Sponsor by the date specified in the notification, or fails to accept the prize, the prize will be forfeited in its entirety and an alternate winner will be randomly selected from the remaining eligible entries. Odds of winning depend on the total number of eligible entries received. The winning participant and their employer will be required to execute and return an Affidavit of Eligibility/Release of Liability/Assignment of Rights/Publicity Release (where legal) and a Deed of Donation document within 14 days of the date of notification. If the Winner fails to do so, the prize will be forfeited in its entirety and an alternate winner will be selected from the remaining eligible entries. The Winner must warrant that she or he is not a Healthcare Professional, a US federal or state government employee, nor prohibited by employment, contract, or law from entering the Sweepstakes or accepting a prize from the Sponsor.
SWEEPSTAKES CANCELLATION: There must be a minimum of ten (10) eligible entries received by the deadline in order to proceed to the drawing. The Sweepstakes will be cancelled unless at least ten (10) entries are received. Should the Sweepstakes be cancelled, the announcement will be posted at thermofisher.com/winminiamp and an e-mail will be sent to the participants to notify them of the Sweepstakes cancellation.
PRIZE: The Winner will receive an Applied Biosystems MiniAmp™ Plus Thermal Cycler (retail value approximately $3995 USD). No cash or other substitutions are permitted, except by the Sponsor, which reserves the right to substitute the prize with one of equal or greater value. No refunds or credits are allowed. Prize is not transferable. TAXES, INCLUDING SALES, FEDERAL, STATE, AND LOCAL TAX, IF ANY, RELATED TO THE PRIZE ARE THE RESPONSIBILITY OF THE WINNER AND SPONSOR WILL, IF APPLICABLE, ISSUE AN IRS FORM 1099-MISC TO THE WINNER.
By entering the Sweepstakes, each participant unconditionally accepts and agrees to comply with and abide by these rules and the decisions of the Sponsor, which shall be final and binding in all respects.
PUBLICITY: Sponsorwill publish the name of the Winner. The Winner consents to the use by Sponsor of Winner’s name and image in Sponsor’s press releases, announcements, or other marketing literature.
